Atherosclerosis in diabetic patients is responsible of increasing patient mortality 4 times than a non-diabetic patient. Diabetic nephropathy and retinopathy are the most common known vascular complications and resulting in end-stage renal disease and blindness, respectively. Other vascular complications are very common like peripheral arterial disease, coronary and carotid arterial lesions. They are also responsible of high rate of amputation, diabetic foot problems and decreasing life expectancy 10 years when compared to non-diabetic individual. Active control of diabetes and early detection and treatment of these lesions assure a better life expectancy and less amputation. Surgical treatment was always complicated by high failure rate due to the nature of the diabetic lesions, delayed wound healing and decreased immunity in diabetic patients. Endovascular techniques was initially dedicated to big size arteries, but recently advanced technology permitted the realization of very low profile tools that can pass through minute lesions in small tibial arteries most commonly affected by diabetes. The purpose of this work is to show the magnitude of the problem, best ways of early detection, its clinical manifestations as well as these most recent vascular techniques and their results in literature.
Khaled Abdel Aal, French vascular surgeon starting vascular surgery career 30 years ago. He is Member of many international societies on vascular and endovascular surgery worldwide. He has created a department of vascular surgery in the national institute of diabetes in his country of origin (EGYPT) which became the biggest center to treat vascular diabetic lesions in Egypt. He also published and contributed in more than 25 papers and recently moved from France to create a vascular surgery department in Al Noor Hospital, Al Ain , UAE.
