Evolution of the endovascular treatment of postinfarction ventricular septal defects

Abstract :

Аccording to the WHO, coronary artery disease, including acute myocardial infarction (MI) and its complications and every year there are ≈ 1.8 million people died from myocardial infarction. It constitute 20% of all deaths in the world. Analysis of the data of the State Statistics Committee of the Republic of Azerbaijan showed that mortality from diseases of the circulatory system, including MI, for 2011-2013. Accounted for 61.1 - 63.8% of the total mortality of the population. Postinfarction VSD is a formidable complication, which accounts for 15% - 20% in the structure of myocardial ruptures , and 0.3-3% complicating of the natural course of acute myocardial infarction. Postinfaction VSD is more common in men than in women, in a ratio of 3: 2. The average age of patients with VSD complicating acute MI is 60 years (from 44 to 81 years).Nevertheless postinfarction VSD is descent after the introduction of thrombolytics and interventional therapy, mortality among patients with postinfarction VSD remained practically unchanged, remaining in the range of 41-80% in the absence of rupture correction. So, on the first day after VSD rupture mortality is 24% -25%, during the first two weeks - 65%, within 8 weeks - 70-92%. Only 7% of patients survive to 1 year .The last group of patients is an exception and is associated with small VSD .
