Mucinous Cystadenocarcinoma appendix and HIPEC- our experience at Pakistan Institute of Medical Sciences

Abstract :

Abstract Primary tumors of the appendix are rare and are usually diagnosed after appendicectomy for appendicitis. A 63 years-old immunocompetent man presented with right flank mass and fever. Abdominal CT showed a Right psoas abscess. Extraperitoneal drainage was performed multiple times for the abscess but it never resolved completely. Tissue biopsy was then sent and histopathology revealed mucinous adenocarcinoma of the appendix. After further workup , laparotomy and Right hemicolectomy with ileocolonic anastomoses was performed. Extensive dissection of tissues, omentectomy, peritonectomy and removal of the lateral abdominal muscles debris was done under the impression of colon cancer. Hyperthermic intraperitoneal chemotherapy (HIPEC) with Mytomycin C was administered for the very first time in our institution. The HIPEC protocol was performed using an open sterile circuit. The total procedure time was 8 hours. The patient was monitored for HIPEC related complications post operatively. He suffered from nausea and reduced appetite which was managed. Dietary advice was given by a nutritionist and supplements were added to the diet. Length of hospital stay was 7 days. Patient was followed up in out patient clinics every month for 3 months. Physical examination, bloods and tumor maker were done which were all normal. Repeat CT scan Abdomen and Pelvis at 6 months did not show any abnormality or recurrence of the disease or metastasis.

She has completed my MBBS in 2014 from Frontier Medical and Dental College, Abbottabad and Fellowship (FCPS) training in General Surgery from Pakisatn Institute of Medical Sciences, Islamabad.
