Abstract :

Objectives: The main objective of this clinical case presentation was to find the cause of a patient who came at the consultation for many, very big nodular formations from hair skin of the head. Material & Methods: Present the clinical case of a woman patient 62 years old who came at the consultation for the apparition with four years ago of many nodular formations from the hair skin of the head and the patient try to cover thesewith the hair but was very difficult. These nodular formations were very big with diameter 8/9 cm, with regular border, smooth area, soft consistence, fixed on the plan of the skin of the head, pink color and with vascularization see on the skin, without pain at the palpation. These formations weren??t present in other part of the body. Other members from family weren??t presented with these similar formations. Mentioned that the patient was born, grow up and live in an area with uranium mines. Except these formations the objective examinations was normal. The laboratory tests were normal except the increase level of ESR=30/40 at 1hour and 2hours and the level of fibrinogen=323mg%. The diagnosis of the patient was difficult to be exact predicated in the first moment without histopathology examination but theoretical could be lipomas, sebaceous cysts from the hair skin of the head or other possible etiology. In the first moment the patient refused surgical intervention but after that she realized that this is necessary and accept. Aftersurgical intervention was performed these looks macroscopic inside with fatty like in lipomas but histopathology examination confirmed unexpecteda carcinoma of the skin??basal cell carcinoma. Results and Disscusions: This clinical case is surprising because the situation is rare and the areas with uranium were the patient live could be a risk factor for these nodular formations. Conclusions: Inthe first instance, without histopathology examination, the real diagnosis was difficult to be established. The case is rare and this is the first clinical case from my medical practice with this clinical presentation. The notification is how many formations appeared in same time in this area with hair of the head, very big and uncomfortable for the patient and with fast evolution in growing up, the long time duration without surgical solution because she afraid and the uranium risk factor from area where she lived. The peoples must to be informed and prevent about the risk of areas with uranium mines very dangerous in developing cancer. The protocol of therapy was completed after surgery with radiotherapy and chemotherapy with good evolution of the patient without recidivate after three years evolution and monitorization.
