Abstract :

Patients who are hyperglycemic following surgery or trauma have worse functional recovery and increased mortality rates. In surgical patients, perioperative hyperglycemia which is often observed, inreases risk of cardiovascular, respiratory, neurologic, and infectious morbidity and it is often undiagnosed. Anesthesiologists as a member of the operating theatres help in identification of diabetic (or hyperglycemic) patients and initiate their treatment intraoperatively. Additionally Intensive insulin therapy (IIT) has been shown to reduce : ? Morbidity and mortality among the critically ill, ? Decrease infection rates ? Improve survival after cardiac surgery, improve outcomes in acute neurologic injury and acute myocardial infarction. However, there has been considerable controversy over the safety and efficacy of IIT. Anesthesiologists wish to keep intra- and postoperative glucose levels somewhere between intensive control and overt hyperglycemia and additionally avoid hypoglycemia.
