Background The advancement of microsurgery has pushed boundaries further. In breast reconstruction, the aim is to reduce complication and improve the outcome. The choice of recipient vessels is crucial for a successful reconstruction. The utilisation of IMAP and IMVP can be a great choice. However, they have certain challenges such as anatomical location and calibre. In particular, delayed reconstruction post radiotherapy cases. Methods: We conducted a systematic review following the PRISMA guidelines. Literature search was performed across PubMed, Trip, Medline, and the Cochrane Library, covering studies published from January 2010 to March 2025. Both authors independently screened the titles of the retrieved articles, excluding irrelevant studies. Abstracts of the selected articles were then reviewed to identify studies suitable for full-text assessment. Additional studies were excluded based on the predefined inclusion and exclusion criteria. Data were extracted from the final selected articles and subsequently analysed. Results: Eight articles were included for thorough analysis out of the167-identified articles from the literature search. The use of IMP recipient vessels in free f lap reconstructions is a viable option with a high success rate (100% flap survival) and a relatively low complication rate (7%). Abdominal flaps were the most commonly used, and the majority of anastomoses were performed using hand sutures for arteries and couplers for veins. The 2nd and 3rd intercostal spaces were the most common sites for IMP vessel use. Conclusion: IMP are reliable recipient vessels and have numerous advantages. However, further studies concerning their utilisation in delayed reconstruction post radiotherapy are needed.
