Comparative study of mucosal flaps in endonasal dacryocystorhinostomy and study of anatomy of nasolacrimal system in human cadavers

Abstract :

Endonasal dacryocystorhinostomy is a highly rewarding procedure for the management of chronic dacryocystitis when the epiphora does not respond to repeated syringing or medical management. The creation of mucosal flaps further improves the chances of success of this endonasal procedure. Forty patients between the age group of 4-77 yrs with chief complaints of excessive watering from the eye were selected from Dayanand Medical College and Hospital Ludhiana . All the cases in the present study were examined based on symptomatic relief from epiphora , post operative size of dacrostome and various other parameters of the dacrostome at 7th day, 14th day, 1 month, 3 month and 6 months follow . Anteriorly, posteriorly, superiorly and inferiorly based nasal mucosal flaps were utilized.The size and other parameters of the dacrostome were graded according to the DCR ostium scoring system. 10 human cadaveric heads were dissected to study the anatomy of nasolacrimal system and measurements were made to quote the relations with the neighboring structures. Conclusion & Significance: The present study yielded a success rate of 90% (36/40). in our study better results were obtained with posteriorly based flap owing to the ease with which it can be raised. Cadaveric dissection is a useful tool for better understanding of the anatomy for refinement of endonasal procedures.
