Abstract :

Most common skin discoloration in North-East is Vitiligo, an acquired familial non- neoplastic depigmentary disorder of skin, hair. Due to selective destruction of melanocytes, which subsequently destroyed and affected area turns pale and becomes white. Evidence of clinical association of vitiligo with AID of organs like pernicious anemia, thyroid disease, IDDM is noted. Eight subtypes are generalised, focal, acrofacial, mucosal, segmental, lip-tip, universalis, unclassified with 30% with minimal type (<2% body surface involvement), 70% extensive type (>5%). In 75% patients, cosmetic success rates observed with split-skin grafting and 35% with epidermal blister grafting which is most commonly used in north-east. 10% used punch graft followed by PUVA or topical fluocinolone acetonide found 80% repigmentation in both cases. 60% success rates in patients under 20 years and higher in segmental or focal type. Relapse rate more common in progressive than in stable vitiligo. Five patients have >75% repigmentation with laser abrasion or dermabrasion with application of autologous disaggregated epidermal cell suspension and NB-UVB or PUVA and followed for 18months. Programmed diathermy (TIMED surgery) and use of skin prepation using erbium-YAG laser has > 95% repigmentation but stil yet to be explored in North-East.
